The city of Salem (Massachusettes?) requires proof of COVID-19 vaccination to enjoy restaurants, indoor entertainment & and attractions and fitness centers, and masks indoors
~ effective January 15, 2022: 12+ must have proof of vaccine
~ effective March 1, 2022: 5+ must have proof of vaccine
~ acceptable proof of vaccination:
– CDC Vaccination Card
– Official Immunization Record
– Digital Image of vaccine record
